TheGonzaFit Resident Fitness Workshop
Terms, Assumption of Risk & Participation Agreement
By registering for and participating in this workshop, I acknowledge and agree to the following:
1. Voluntary Participation
I understand that participation in the fitness workshop hosted by TheGonzaFit is completely voluntary. I may choose not to perform any exercise, movement, or activity at any time.
2. Physical Activity & Assumption of Risk
I understand that physical exercise and the use of fitness equipment involve inherent risks, including but not limited to muscle soreness, strains, sprains, falls, dizziness, cardiovascular events, aggravation of an existing condition, and other injuries.
I voluntarily choose to participate with knowledge of these risks and accept responsibility for risks ordinarily associated with physical exercise and fitness activities.
3. Health Responsibility
I understand that TheGonzaFit does not provide medical diagnosis, medical treatment, physical therapy, or individualized medical advice.
I am responsible for determining whether I am physically able to participate. If I have a medical condition, injury, symptoms, physical limitation, or uncertainty about my ability to exercise safely, I understand that I should consult an appropriate healthcare professional before participating.
I agree to stop exercising and notify the instructor if I experience unusual pain, dizziness, shortness of breath, chest pain, or other concerning symptoms.
4. Instruction & Personal Responsibility
I agree to follow reasonable safety instructions provided during the workshop and to use fitness equipment only in the manner demonstrated or intended.
I understand that the workshop provides general fitness education and is not a substitute for individualized personal training, medical evaluation, or rehabilitation services.
5. Liability Acknowledgment
To the fullest extent permitted by applicable law, I accept responsibility for injuries or damages resulting from the ordinary and inherent risks of my voluntary participation, my failure to follow instructions, my misuse of equipment, or my own actions.
Nothing in this agreement is intended to waive or limit any legal right or claim that cannot lawfully be waived under Massachusetts law.
6. Avalon at Chestnut Hill
I understand that the workshop is being conducted at the Avalon at Chestnut Hill fitness center.
I agree to follow all applicable property, fitness-center, equipment-use, and resident rules while attending the event.
TheGonzaFit is independently hosting the workshop and is not an employee of Avalon at Chestnut Hill.
7. Emergency Assistance
If I become injured or experience a medical emergency during the workshop, I authorize the instructor or property staff to contact emergency medical services when reasonably appropriate.
I understand that I am responsible for medical expenses arising from any treatment I receive.
8. Limited Registration & Attendance
I understand that workshop capacity is limited so participants can receive appropriate instruction.
By reserving a spot, I agree to make a reasonable effort to attend. If I can no longer attend, I agree to cancel my registration or notify TheGonzaFit as soon as reasonably possible so the spot may be offered to another resident.
Failure to attend does not create any financial obligation or penalty.
9. Personal Information
Information submitted during registration may be used by TheGonzaFit to administer the workshop, confirm registration, send event reminders, and provide workshop-related resources.
Your email will be used to send your registration confirmation, event reminders, and workshop-related resources.
10. Age Requirement
Participants must be at least 18 years old unless participation by a minor has been specifically approved and a parent or legal guardian must be present.
11. Conduct & Safety
TheGonzaFit may ask a participant to stop an activity or leave the workshop if the instructor reasonably believes continued participation creates a safety risk to that participant or others.
12. Governing Law
These terms are governed by the laws of the Commonwealth of Massachusetts.
If any provision of these terms is determined to be invalid or unenforceable, the remaining provisions will continue to apply to the extent permitted by law.
Acknowledgment
By Signing in and submitting my registration, I confirm that:
I have read and understand this agreement.
I voluntarily choose to participate.
I understand the risks associated with physical exercise.
I agree to the participation terms above.
I understand that checking the box constitutes my electronic acknowledgment of these terms.
